Concentric, providing onsite therapy and wellness services for aged care
Concentric began as a piloted innovative model of care in 2017, as Australia's first rehabilitation centre co-located within a residential aged care facility, delivering enhanced physiotherapy and allied health services to both facility residents and those in the wider community.
Due to overwhelming success. Concentric have gone onto co-locate within aged care facilities across the country and continue to expand rapidly providing onsite therapy to residents and the local surrounding community.
There are four components to our suite of services that are delivered from a dedicated Wellness Centre on site at the residential aged care facility, or as an allied health visiting service.

- External Client Services: rehabilitation and allied health services for the general public
Concentric proudly partners with many national aged care approved providers, including industry leaders Estia Health and Opal Healthcare, as well as state-centric providers like Homestyle Aged Care who are based in Victoria.
CORE SERVICES
Concentric delivers core services to the residential aged care residents incorporating all Initial Assessments, Functional Assessments, Care Plan Reviews and Short Term Booster Programs aligning to the AN-ACC funding model. Our practitioners will work closely with the residential clinical team to ensure effectiveness of the programs and compliance with AN-ACC and the Aged Care Quality Standards. Education and manual handling training can also be provided to staff on request as part of the core service hours. In addition to the assessments, Concentric are able to provide an evidence based booster program that is run over an 8 week duration and consists of interventions delivered in group classes or one-on-one streams to relevant residents. These programs are standardised protocols targeting the below:
Stand Strong: Falls Prevention (Group)
Take Charge: Pain Management (Group or 1:1s)
Bounce Back: Physical Restorative Care (Group or 1:1s)
Active Minds: Physical + Cognitive Restorative Care (Group or 1:1s)
Connect 4: OT Specific Cognitive Restorative Care (Group or 1:1s)
Additional allied health services for residents
Concentric delivers additional allied health support in a multidisciplinary approach, in the disciplines of physiotherapists, occupational therapists, exercise physiologists, speech pathologists and dietitians. These support services can be in the form of additional one-on-one sessions and are ideal for targeted or intensive rehabilitation. Residents are able to utilise these services on top of the Wellness Packages or as stand alone services. They can utilise other funding options such as private health insurance or medicare rebates with this service.
Wellness packages for residents
A balanced Wellness Package is structured to address limitations in pain, functional capacity and other conditions. Every resident who chooses to be on the Wellness Package receives a comprehensive assessment by a Concentric allied health professional. The outcomes of the assessment are communicated to the clinical team and the family. Based on the outcomes of this assessment, the resident will then receive recommendations about suitability of each stream of the Wellness Packages outlined below:
Falls & Balance
Tailored to improve balance and function to reduce the risk of falls.
Targets:
• Balance
• Social interaction
• Cognitive function
• Physical function
Strength & Mobility
Tailored to residents with a reduction in their range of motion or reduction in their participation in activities of daily living. For example orthopaedic conditions including rotator cuff tears and post surgery.
Targets:
• Reduction in pain
• Improved range
• Increased strength
• Physical function
Functionality
Tailored to residents who require assistance with mobility and transfers. For example neurological conditions including Parkinson's, MS & post stroke.
Targets:
• Maximising function
• Increasing strength
Our Wellness Package also includes:
• Individually tailored exercise programs
• Unlimited access to the Wellness Centre during supervised hours
• Reassessments every 3 months where their progress and program is reviewed
• Quarterly case conferences with family as requested
