TeleConnect is a service that is led by a senior allied health professional who assesses, recommends, and documents an appointment virtually whilst utilising an Allied Health Assistant (AHA) attends the clientโs home in-person, as required. TeleHealth is a service that is solely delivered by a senior allied health professional virtually.
The flexibility of TeleConnect appointments can also lead to increased user engagement, adherence to treatment plans, and achievement of personalised individual goals.

Allied Health Services Within Residential Aged Care (RAC)

How does TeleConnect work?
Concentric will assign a senior therapist who coordinates the requirement of need through a virtual assessment with the assistance of Allied Health Assistant (AHA).
Step 1: Initial Assessment
A comprehensive virtual evaluation led by our experienced therapist to understand the clientโs needs and set goals. An AHA will be with the client in-person to facilitate any assessment/s that cannot be completed virtually.
Step 2: Personalised Care Plan
Development of a tailored care plan, integrating both virtual and in-person support.
Step 3: Bringing the Care Plan to Life
Our AHAs and therapists work together to execute the care plan, ensuring each client receives the personalised support they need.
Step 4: Comprehensive Reporting
Regular updates and detailed reports to track progress and make necessary adjustments, ensuring optimal outcomes for clients.
