Our dedicated Suport at Home Team will work with you or your loved ones to meet their physiotherapy and occupational therapy needs, including:
- Dedicated Support a Home scheduling team allowing for streamlined referral process and speed to task in response and booking times
- Fully mobile clinical workforce specialising in community in-home treatments
- Clinical team experienced in working with people with dementia, neurological conditions, osteoporosis, arthritis, diabetes and more
- Optional in-home or in-clinic therapy options for suitable clients
- All reports and progress notes sent to Case Manager in a timely fashion
- Approval for recommendations and ongoing follow up to be emailed by case manager before commencing ongoing therapy
- Free allied health education sessions to Case Managers
- Extensive clinical support to ensure highest quality of care
- Single Operational Account Manager
- Regular service reviews and check ins to ensure we are meeting providers expectations
Our mission
Our purpose is to increase access to quality care. We focus on assisting clients to achieve their goals to enable them to remain at home for as long as possible.
We support the following Support at Home funding options:
- Support at Home Packages.
- Privately funded patients.
A Support at Home Package is an Australian government-funded program that provides financial assistance to individuals with age-related care needs, allowing them to receive care and support in their own homes rather than residential aged care. It helps people maintain their independence and live safely at home for as long as possible.
The Support at Home program was introduced in November 2025, replacing the Home Care Packages Program and the Short-term Restorative Care Programme. It improves access to services, products, equipment, and home modifications to help you remain living independently at home for longer.
Support at Home can be an option if you need a coordinated approach to the delivery of your help at home. Perhaps because you need help with many everyday tasks, or the care you need is more complex or intensive.
