Homecare

Our dedicated Homecare Team will work with you or your loved ones to meet their needs, including:

• Dedicated Homecare scheduling team allowing for streamlined referral process and speed to task in response and booking times
• Fully mobile clinical workforce specializing in community in-home treatments
• Clinical team experienced in working with people with dementia, neurological conditions, osteoporosis, arthritis, diabetes and more
• Optional in-home or in-clinic therapy options for suitable clients
• All reports and progress notes sent to Case Manager in a timely fashion
• Approval for recommendations and ongoing follow up to be emailed by case manager before commencing ongoing therapy
• Free allied health education sessions to Case Managers
• Extensive clinical support to ensure highest quality of care
• Single Operational Account Manager
• Regular service reviews and check ins to ensure we are meeting providers expectations

Our purpose is to increase access to quality care. We focus on assisting clients to achieve their goals to enable them to remain at home for as long as possible.

We support the following Homecare funding options:

• Commonwealth Home Support Program (CHSP).
• Home Care Packages (HCP).
• Short Term Restorative Care (STRC).
• Privately funded patients.
